Taking SSI and I want Medicare
You don’t need to do anything. You will automatically be enrolled in Part A & Part B. You will need to add a Medicare Advantage or Medicare Supplement Plan to cover the costs that Part A & Part B don’t cover. Contact us to learn about your options.
Taking SSI and I DON'T want Medicare
You will need to disenroll from Part B. You can download form CMS 1763 to request the termination of your Part B. Make sure to explain why you are disenrolling from Part B. Contact us for any assistance.
NOT taking SSI and I WANT Medicare
You can sign up for Part A & Part B online at SSA.gov. We can walk you through this process. Contact us for assistance.
Not taking SSI and I DON'T want Medicare
You don’t need to do anything. You won’t be automatically enrolled.
I'm over 65 years old and need Medicare. What should I do?
I have Part A and just need Part B
You will need to download and complete Form 40B to request your Part B to begin. You will need to download form L564 and have your employer complete it. This form states you have had credible coverage since turning 65. This will eliminate a possible late enrollment penalty. Once both of those are completed you will mail them to your local Social Security office.
I don't have Part A or Part B
You can sign up for both Part A and Part B online at SSA.gov. You can only sign up online when your Part A and Part B will have the same start date. Contact us for assistance in signing up for Medicare.
